SPEAKER REQUEST FORM (to be used if your invitation includes ANY speaking role at your event)

If you are interested in inviting a Council member, our Executive Director, or one of our staff to speak at your event, please fill out the Speaker Request Form. Requests for PCFSN participation should be submitted at least eight (8) weeks prior to your event to allow ample time for processing and scheduling of your request.

EVENT REQUEST FORM (to be used only if your invitation does NOT include any speaking role at your event)

If you are interested in inviting a Council member, our Executive Director, or one of our staff to attend, exhibit, lead a physical activity or participate in your event in a role other than as a speaker, please fill out the Event Request Form. Requests for PCFSN participation should be submitted at least eight (8) weeks prior to your event to allow ample time for processing and scheduling of your request.

PLEASE NOTE:
Some users have reported experiencing problems trying to submit the Speaker and Event Request Forms using Internet Explorer.  We are currently working to correct this problem and apologize for any inconvenience you may experience in trying to submit your form.  In the interim, we recommend that you use a browser other than Internet Explorer when submitting your Speaker or Event Request Form. 
Once you fill out a Speaker or Event Request Form and click the "Submit" button, this should automatically generate a response e-mail which you should receive shortly after submitting your form.  This e-mail will also contain a copy of your submitted form for your records.  If you DO NOT receive this automatically generated e-mail then your form did not submit and you should resubmit the form.  If you continue to experience problems submitting your form please contact our Deputy Director of Communications, Joey King, at russell.king@hhs.gov to report this problem. If for any reason you are unable to submit your form via our website, or only have access to Internet Explorer as your browser, please contact Mr. King to request that he e-mail you a Word version of the form that you can submit as a alternate means of requesting PCFSN participation at your event. Any questions related to our Speaker Request Form and Event Request Form, or about our invitation process in general, can also be directed to Mr. King at the e-mail address listed above.
